About the role
The ACAT Assessor position stands within the of the CCH/ACAT multidisciplinary team. The primary purpose of the position is to provide holistic comprehensive assessment, support and management to the target population in the St George Geographical area in the community, hospitals and aged care facilities as applicable.
ACAT Assessments completed in nominated Key Performance Indicator (KPI) Timeframe
100% accurate documentation in the My Aged Care System and the Electronic Medical Record
Maintain CPD points as per AHPRA registration requirements
Completion of Mandatory Training
