About the role
As Care Coordinator you will provide complex care coordination to clients across various funding streams.
As a member of our rostering team, you are an experienced, dedicated, and passionate scheduling coordinator. You will help rostering the support for our clients, assisting with their current needs at home and in the community, using our client management system.
The role involves communicating with our clients and or family, regarding their services and support needs, ensuring they are rostered to meet their needs.
We are looking for a Care Coordinator to join our friendly, supportive team based in Lambton!
What you bring
Current Drivers Licence
Certificate IV in Community Services or a related field
Experience in Aged Care, Disability or Community Care
Experience with people with Dementia
Previous experience in care coordination
Knowledge and understanding of person centred service delivery
A desire to join a team committed to the care of others in a mission based organisation
