Permanent, full time opportunity to join our Community Care team in providing case manager services to our Home Care clients
About the role
As a Care Manager - Home Care, you will be responsible for delivering case management to our clients that is person centred and enablement focused.
You will be involved in all aspects of care coordination, including developing care plans, facilitating referrals, rostering of client services and completing administrative tasks and reporting. You will develop positive working relationships within the local service centre, and more broadly across the organisation to facilitate improved outcomes for our clients.
About you
You will bring:
Tertiary qualifications in Community Services / Case Management or related discipline
Demonstrated experience in community care case management and experience developing care plans for complex clients
Demonstrated ability to understand and adhere to reporting requirements
High level of computer systems proficiency, including data entry
Highly developed communication and time management skills
A current and valid driver's licence and insured motor vehicle
NDIS Workers Screening Check or willingness to obtain
