About the role

The role is  case managing the care required and the case load of our Home care package Clients. Maintaining budgets on boarding new clients and managing care requirement needs.

Key Responsibilities

• Case management experience
• knowledge of HCP/CHSP funding
• Drivers License and own insured car.
• A team player, within a multi D team
• Office based Job role in Adelaide
• Computer skills
• able to multi task  

What you bring 

• A valid or willingness to get a NDIS clearance
• A Valid working with children check or willingness to get one.
• A desire to join a team committed to the care of others in a mission based organisation
