About the role

As Case Manager Team Leader you will be responsible for providing leadership and direction for the team of case managers in the provision of person centred and reablement focussed service delivery within a budget framework.  You will utilise your skills and experience, and be proactive in managing and prioritising package referrals and leads in accordance with policy and defined practices. 


What you bring 

  • Relevant tertiary qualification/s and experience in health/aged/disability/home care sector
  • People leadership and management skills and capability
  • Ability to communicate effectively and work through change management.
  • Experience to work in a multidisciplinary team, in collaboration with employees within the organisation and with external agencies and personnel.
  • Ability to build and maintain strong partnerships with external stakeholders.
  • Knowledge of the scope of practice of a Case Manager and Intake in the community setting
  • Demonstrated ability to assess and develop innovative strategies to support people requiring assistance to remain living independently within their own homes.
  • Commitment to promoting and facilitating independence for those clients receiving our services including the use of technologies.
  • Demonstrated ability to work within the program guidelines of funded services, regarding assessment, service purchase/provision, reporting and accountability and budgetary management.
  • Current COVID-19 Vaccinations including booster
  • A desire to join a team committed to the care of others in a mission based organisation
