About the role
Based in Adelaide CDB, we are looking for a full time Clinical Care Coordinator who will be responsible for coordinating clinical services in order to support clients in their own home.
Working as a team, you will be the primary point of contact in supporting clients and staff to ensure services are delivered efficiently and provide or clients with the best possible outcome.
Your main duties willinclude:
Daily coordination of care for new and existing clients of the SA Health Community Care Program.
Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available to commence service delivery within the required service delivery timeframes.
Identify clients in the clinical programs that are eligible for other Government funding streams and liaison with the referrer, MRU or healthcare professional for transfer of care.
