Registered Nurse  / Registered Psych Nurse – Full time or Part Time role will be considered

About the role

We are seeking an experienced Registered Nurse or Psychiatric Nurse to join our Movement Disorders Team, as a Clinical Liaison, working within the multidisciplinary team in the Statewide Progressive Neurological Diseases Service. 

This role is an opportunity for a senior experienced nurse to work exclusively with Huntington's disease and other Progressive movement disorders and use highly refined clinical and communication skills  to support people living with these complex and challenging conditions.  Alternatively, this is an excellent opportunity for a less experienced nurse to develop skills and expertise in the areas of case management and complex conditions that require the development of expertise, drawing on and developing a knowledge of psychology, cognition and complex physical issues whilst working in a supportive team environment to hone and refine emerging clinical and communication skills

Key Responsibilities (what your day might look like)

• Attending neurology, psychiatry, and/or palliative care clinic appointments, via telehealth or in clinic face-to-face, to ensure continuity of care
• Arranging follow up referrals, appointments, care plans and management strategies arising from clinic appointments
• Telephone triage and assessment of needs with patients, families, and care providers
• Undertaking interdisciplinary appointments with patients to complement the medical appointments
• Providing specialist advice and support to patients, family members, and care providers over the telephone, via telehealth or in face-to-face clinic appointments
• Participating in multi-disciplinary team meetings and informal discussions

What you bring 

• AHPRA registration as a Registered Nurse by the Nursing and Midwifery Board of Australia or Allied Health Professional equivalent based on clinician specialty
• Experience in, or a demonstrated interest in, one or more of:
  • Progressive Neurological Diseases, Palliative Care, Mental Health, Disability,
  • Community Health, care planning and care coordination
  • Management of patients with complex psychosocial, physical and/or mental health needs
• Ability to work independently and autonomously in complex situations, within scope of practice, and respond to unknown and/or unexpected dilemmas whilst demonstrating understanding, respect, and applying principles of dignity of risk to maximise quality of life
• Ability to form therapeutic relationships with patients, families and carers enabling provision of psychosocial and emotional support
• Motivation, enthusiasm and energy and a desire to embrace opportunities for participating in research, education and ongoing quality improvement activities and a desire to join a team committed to the care of others in a mission based organisation
