 About the role

Clare Holland House is looking for a Palliative Care Nurse Practitioner who will be guided in their practice by the ANMC competencies and the National Standards of Palliative Care. This role is within the PEACE team who delivers in-reach palliative care services to all residential aged care facilities (RACF) in the ACT.  This team is nationally recognised for developing and implementing the Palliative care Needs Rounds model of care.  The RN 4.2 will provide advanced clinical assessment, develop and evaluate care plans in collaboration with the patient their family liaise closely with GP, Care mangers and other members of the multidisciplinary team.

What you bring 

  • Endorsed Nurse Practitioner
  • Act as a professional role model and resource person in the provision of active, compassionate patient centered care as a member of the interdisciplinary team
  • Provide specialist clinical palliative care services to palliative patients and their families within the ACT RACF setting including the provision of expertise in assessment, symptom management and end of life care
  • Directly provide comprehensive and coordinated patient centered care (acknowledging the patient and their significant others as the unit of care) utilising processes of continuous assessment, negotiated support and evaluation in accordance with the ANMC National Competency Standards and the National Standards of Palliative Care
  • Participation in research and professional learning to enhance models of care for residents in Residential caged care facilities
