About the role
Full time temporary role in community palliative care (backfilling a 13 months maternity leave)
Based at Calvary Health Care Kogarah
You will be a passionate social worker interested in working on a dynamic multi-disciplinary team caring for people with life limiting illness.
Under the guidance of a Level 3 senior social worker you will provide social work assessments and interventions for clients of the Calvary Community Palliative Care Team in St George and Sutherland LGA's.
Your main duties will include:
Providing psychosocial assessments, counselling, support, advocacy, referrals, family/carer support, information and psycho-education
Providing services to people in their home or community setting
Liaison with treating team and other service providers
About you
Bachelor of Social Work or equivalent with eligibility for membership of AASW
Current Driver's License
Experience in hospital social work preferably in palliative care, oncology or aged care
Experience working with diverse populations
Interested in working in a social work Department that values professional development, learning and supervision
