About the role
Calvary Home Care - Adelaide is looking for a new HCP case manager to support the growing team. The role is office based Monday- Friday in the Adelaide CBD.
This exciting position is to support with new intake and will support onboarding new HCP clients and also manging their care needs.
You will be working in a team of 6 Case managers and also working directly with the intake officer.
Your main duties will include:
HCP Intake and Community engagement with new clients
The management of HCP funds and Services
The management of client care needs in the community
About you
An enthusiastic attitude
Understanding of Home care package funds
Understanding of the aged care reform
People Skills
Experience in Case management or Social Work
