About the role
The role is to manage a case load of clients under CHSP/HCP approved funding. Maintaining budgets, on boarding growth of new clients and managing care requirements to maintain clients independency.
Key Responsibilities
- Time Management
- Understanding the Aged Care Standards
- Liaising with stakeholders and internal networks
What you bring
- Evidence of a minimum of 1 years experience in Case Management
- Relevant tertiary qualification in the community sector
- Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
- Full Drivers Licence and own reliable registered and insured Vehicle
- Smart Phone
- Knowledge of My Aged Care portal/assessment process
- Willing to work in a team environment
- A desire to join a team committed to the care of others in a mission based organisation
- NDIS clearance (or willingness to obtain one)
- Police clearance (within the last 6 months Or willingness to obtain one)
