INTAKE OFFICER - CALVARY HOME CARE

About the role
This full time role involves processing referrals, assessing client needs, and coordinating services to ensure a smooth and appropriate intake process. The Intake Officer acts as a key point of contact for clients and plays a crucial part in connecting individuals with the necessary in home support services.
Your main duties will include:
- Processing and managing referrals through My Aged Care
- Evaluating client needs through phone screening and assessments
- Providing information about Commonwealth Home Support Programme (CHSP)
About you
- Ability to take initiative, work independently and as part of a team
- High level of attention to detail and strong organisation and communication skills
- Efficient, confident and hardworking
- Experience within the aged care sector
- Understanding of CHSP and other relevant programs
