Clinical Care Coordinator – Neurosurgery

RN level 2 - 0.8 FTE

Are you an experienced Registered Nurse looking to broaden your career in clinical care coordination, case management, and complex discharge planning?

An exciting opportunity exists for a motivated and clinically experienced nurse to join our Neurosurgery team as a Clinical Care Coordinator at Calvary Adelaide Hospital. Working four days per week (Flexible across Monday to Friday), commencing September 2026, this role offers a unique opportunity to work collaboratively across multidisciplinary teams while making a significant impact on patient outcomes and the overall patient journey.

Reporting jointly to the Clinical Care Coordinator Manager and Neurosurgical Clinical Manager, you will play a key role in coordinating patient care from admission through to discharge, ensuring safe, timely, and efficient transitions while supporting optimal length of stay outcomes.

This position is ideal for a nurse who enjoys building strong professional relationships, coordinating complex care, and working closely with surgeons, medical teams, allied health professionals, and ward staff to achieve the best outcomes for patients.

What You’ll Bring:

You will be an experienced Registered Nurse with sound clinical knowledge, excellent communication skills, and a passion for coordinating patient-centred care. You will have a strong understanding of acute surgical care, discharge planning, and the ability to work collaboratively across multiple stakeholders.

Applications are invited from Registered Nurses with a minimum of three years acute clinical experience who are interested in further developing their skills in case management, care coordination, and discharge planning.

Requirements:

  • Registered as a nurse with AHPRA and holds a current practicing certificate.
  • Highly developed clinical skills integrated with highly developed interpersonal skills.
  • Ability to plan and establish work priorities with a high level of attention to detail.
  • Proven organisational skills and the ability to handle multiple tasks.
  • Commitment to excellence in customer service and continuous quality improvement
  • Willingness to engage and promote a positive ward culture
  • Knowledge of varied health funds and length of stay expectations

Desired Experience:

  • Neurosurgical nursing experience
  • Knowledge of health fund requirements, DRGs, and length of stay expectations
  • Experience in care coordination, discharge planning, or case management
