  • About the role

    As the Clinical Care Coordinator you will coordinate and lead the care team in consultation with the Home Manager. You will ensure compliance with legislative and statutory requirements to enable us to continue to deliver a high standard of care and most importantly, a quality lifestyle for our residents.

    Working within a supportive and dynamic team, you will be provided with a challenging and diverse work environment, as well as an opportunity for you to contribute and make a real difference to the residential aged care community.

    We currently have a full time permanent opportunity for an experienced Clinical Care Coordinator for our facility in The Mariner, South West Rocks.

     
    Your main duties will include:
    • Coordinate the delivery of quality clinical services in line with a resident centred model of care
    • Maintain current knowledge and understanding of Aged Care Funding models
    • Manage and provide efficient, effective and compassionate care planning and assessment
    • Coordinate the availability of medical stock and other health related supplies to ensure sufficient and adequate supplies
    About you
    • Registered Nurse with current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) 
    • A commitment to work within Calvary’s Mission, Vision and Values
    • Demonstrated knowledge and understanding of the legislation governing the Aged Care
    • Commitment to contemporary aged care nursing practice 
    • Ability to support a multi-disciplinary team  Effective communication skills, oral and written
    • Demonstrated alignment with the philosophy of resident focused care which supports dignity, independence and informed consultative decision making
