Palliative Care Clinical Nurse – Calvary Palliative Home Care Team

About the role
The Clinical Nurse is responsible for providing safe, effective and appropriate patient care in a patient-focused environment.
Working 4 days per week across a 7-day rotating roster, you will provide high-quality palliative care to patients in their homes, supporting them and their families through all stages of care. You will practice in accordance with legislative requirements and the mission, values, policies, and procedures of Calvary.
This role offers the opportunity to work collaboratively across the palliative care service, including engagement with the Mary Potter Hospice team where required.
Key Responsibilities
- Deliver comprehensive, patient-centred assessments and care planning
- Provide high-quality palliative care in the home environment
- Monitor, evaluate, and adapt care plans in response to changing patient needs
- Collaborate effectively with GPs, Nurse Practitioners, and the multidisciplinary team
- Work autonomously while managing a community-based caseload and home visits
- Maintain accurate clinical documentation and follow-up care
- Work within WHS requirements specific to community nursing
- Contribute to education, mentoring, and support of staff
- Demonstrate leadership, accountability, and commitment to continuous improvement
What you bring
You are a motivated and compassionate clinician who brings a positive, proactive approach to care. You are confident working independently in the community while remaining closely connected to your team, and you are committed to delivering high-quality, person-centred palliative care.
- Current registration with AHPRA as a Registered Nurse
- Minimum 12 months post graduate experience in palliative care
- Demonstrated experience in a community or home-based clinical nursing role (highly desirable)
- Ability to work autonomously and manage competing priorities in a dynamic environment
- Strong communication and interpersonal skills, with the ability to build therapeutic relationships
- Proven ability to work effectively within a multidisciplinary team
- Sound knowledge of the National Safety and Quality Health Service (NSQHS) Standards
- Commitment to the Mission and Values of the Little Company of Mary
- Current, unrestricted driver’s licence
- Proficiency in Microsoft Office applications
- Postgraduate qualification in Palliative Care, or willingness to work towards
