 About the role

We are looking for a Clinical Nurse Specialist Grade 2 to join our team at Kogarah. 

As a Clinical Nurse Specialist Grade 2 you will provide specialised, individualised, case managed complex client care and clinical leadership for the Transitional Aged Care Service (TACS) at Calvary Health Care Kogarah (CHCK) in order to maximise outcomes for patients/clients and their carers consistent with Calvary, NSW Ministry of Health and relevant South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

The position operationally reports to the TACS Manager and professionally to the Rehabilitation and Palliative Care Services Nurse Manager.

You will be an integral part of the multidisciplinary team and will lead, support and supervise other nurses within the team. You will strive to ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate.

Selection Criteria:

  • Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse and current unrestricted Class C NSW Driver’s License and willingness to travel as required in role.
  • Relevant post-registration qualifications and at least 3 years’ experience working in aged care, rehabilitation or community setting, or such other qualifications or equivalent experience deemed appropriate.
  • Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans.
  • Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
  • Demonstrated skills in the development and facilitation of patient and carer education; and ability to mentor and coach nursing staff to enhance professional development.
  • Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
  • Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
  • Demonstrated competence in the use of health-related information technology.
  • Ability to work within the Mission and Values of the Little Company of Mary Health Care
