About the role
 
You will be working with a skilled, supportive team providing Palliative Care to people at home and in aged care.
You will work independently but will have strong support networks and extensive orientation 
You will be undertaking symptom management and assessment, admissions to service, consultation, emergency crisis management, goals of care planning and liaising with external health providers. 
We are located Caulfield South.
Role is for 12-month fixed term contract 
 
Your main duties will include:
  • Palliative assessment and management of symptoms, home based palliative care
  • Future planning and goals of care discussions
  • Collaboration with internal and external specialties 
About you
  • Rotating roster, mainly day shifts with some late shifts 
  • Participation in oncall roster (after hours)
  • Current Victorian Drivers licence 
  • License, certificate, degree,
  • Current APHRA Nursing registration 
  • Minimum 3 years post grad experience
  • Post grad in Palliative Care desirable but not essential 
  • High level of commitment, integrity and honesty.
