About the role
This role plays a vital part in ensuring safe, timely, and effective discharge planning for patients. Working as part of a multidisciplinary healthcare team, the Discharge Planner supports patients and their families by coordinating care, services, and resources to promote continuity of care following discharge. This is a full time relieving role Monday to Friday.
Your main duties will include:
- Assessing patient needs and discharge readiness
- Coordinating community services and support agencies
- Collaborating with clinical staff, patients, and families
- Advocating for patient-centred, safe discharge outcomes
About you
- Current AHPRA registration
- Relevant clinical knowledge, skills and experience developed in an acute care environment
- Willingness to educate and provide advice to patients and nursing staff
- Demonstrated critical thinking, problem solving and planning skills
- Demonstrated advance communication and interpersonal skills relating to internal and external customers
- Experience in discharge planning
