About the role
Based at Calvary Kooyong in Caulfield South, Calvary Healthcare Bethlehem is a leading provider of specialist services for people living with progressive neurological diseases and those needing palliative care. Services include an inpatient ward, Community Palliative Care Service, State Wide Progressive Neurological Diseases Service, and NDIS Provider service.
You will work across our service streams, aligning with your experience and skillset. The vacant role will commence on the inpatient unit, with rotation options to other service streams as opportunities arise in the team.
You will provide Occupational Therapy services to people with progressive neurological diseases (PND) or other life limiting conditions and their formal and informal support network to support living well. You are part of a supportive and compassionate multidisciplinary team including a wide range of allied health professionals, medical consultants, specialist nurses and external stakeholders. Role will include predominantly hospital-based work and home-based assessments as clinically required.
Your main duties will include:
- Occupational Therapy assessment and intervention to people living with life limiting conditions (e.g. palliative, MND, HD)
- Supporting discharge planning aligning with patient-centred goals
- Providing carer training to support meeting of care needs on discharge
- Contributing to pressure care and falls risk management plans
- Providing assistive technology for use on the ward and planning discharge needs
- Attending meetings (family meetings, discharge planning, case conference, handover)
- Working with other team members closely, particularly AHA, Physiotherapy, Speech Pathology, Nursing and Social Work
- Supporting NDIS access and navigation of NDIS and aged care systems
- Working with internal and external stakeholders to provide compassionate care
- Participation in OT department operational and quality meetings
- Documentation of patient notes and reports, development of assessment reports and discharge summaries

- An OT who enjoys supporting quality of life and problem solving creative solutions
- You have some experience or interest in developing skills in progressive neurological OT and/or palliative care
- You have outstanding communication skills

- You are self-directed, organised, enjoy a balance of team and autonomous tasks
- You are reflective in your clinical practice
- You are a person-centred, creative and flexible therapist who is able to work with dignity of risk and respect for individual choice and control
- You have some knowledge in navigating the interface of palliative care, health and disability to support patients and team members with coordination of care
- AHPRA registered Occupational Therapist
- Drivers Licence
