About the role

As a member of the Calvary Community Care team, you will deliver quality services to meet the needs of our clients and contribute to a positive experience whilst at Calvary Community Care. Assist in planning, assessing and evaluating our clients needs with the help of our multidisciplinary team, to promote positive health outcomes. The Registered Nurse provides clinical support to our clients and collaborates with our support workers to deliver exceptional care to our clients in the community. 

Based at Wagga Wagga and services Riverina areas such as Albury, Cootamundra, Gundagai, Tumut, Temora, Junee and surrounds, for clinical and nursing support. This role encompasses working with hospitals, community health centres, Home Care Package case managers and allied Health professionals including liaising with GP's

Key Responsibilities

• work with clients/cares and families to identify clinical goals
• develop and maintain clinical care plans
• work within current Calvary policy and procedures and WHS guidelines
• coordinate and organise the provision of nursing care including clinical comprehensive assessments
• work with HCP case managers and liaise with VHC program/DVA community nursing and other services

About You

• AHPRA Registration 
• Recent nursing/clinical experience in the last 5 years or evidence of a refresher course
• Effective communication, interpersonal and organisational skills
• Initiative and the desire to continually improve client care
• Capability to prioritise tasks and achieve goals within set timeframes
• Candidates with an understanding of the community nursing sector - Home Care Packages, Department of Veteran Affairs and Commonwealth Home Support Program will be considered favourably
• A desire to join a team committed to the care of others in a mission based organisation
