About the role:

Join the Transitional Aged Care Service team at Calvary Kogarah. We are seeking a dedicated Social Worker (Level 1–2) to join our supportive team in a permanent part-time role, working 64 hours per fortnight. This is a great opportunity to make a meaningful difference in the lives of older people as they transition from hospital back to the community.

Your main duties will include:

  • Work as a part of the TACS multidisciplinary team to assist clients achieve optimal outcomes
  • Plan, coordinate and deliver high quality social work care to patients/clients of Calvary Health Care Kogarah Transitional Aged Care Service (TACS

About you

  • Qualifications in accordance with Schedule C of the NSW (Non-Declared) Affiliated Health Organisations’ Professional and Associated Staff Agreement 2022, eligibility for membership to the Australian Association of Social Workers; demonstrated commitment to continuing professional development
  • Current drivers licence.
  • Demonstrated ability to exercise independent core competency skills in social work.
  • Demonstrated high level written, verbal and interpersonal skills, to both engage patients/clients and work effectively with the multi-disciplinary team.
  • Demonstrated ability to exercise independent professional judgement in the provision of clinical care using evidence-based practice, commensurate with years of experience.
  • Demonstrated commitment to the use and application of professional and clinical digital technologies.
  • Demonstrated participation or leadership in quality improvement processes and teaching, commensurate with years of experience.
  • Ability to respond to the demands and pressures of the role while managing time effectively.
  • Ability to work within the Mission and Values of Calvary Health Care Kogarah.
