About the role

Calvary is seeking experienced and passionate Clinical Care Coordinators (CCC) to join our Travelling Workforce Program, supporting Residential Aged Care services across Australia.

This is an exciting opportunity for an experienced clinical leader who enjoys variety, thrives in new environments and is passionate about delivering exceptional resident care while supporting and mentoring clinical teams.

Placements are fixed-term full-time contracts typically located throughout regional NSW, VIC, TAS, SA and ACT with assignment lengths varying between 4 weeks to 12 months based on operational requirements.

As a Clinical Care Coordinator, you will provide clinical leadership, coordinate resident care, support quality improvement initiatives and work collaboratively with Home Managers and multidisciplinary teams to ensure safe, high-quality care.

This is an exciting opportunity to work across a range of aged care homes while developing your clinical skills, meeting new teams, and experiencing different communities throughout the state.

The Enrolled Nurse is responsible for providing safe, effective and appropriate care in a patient-focused environment.

Key Responsibilities

  • Coordinate and oversee the delivery of safe, person-centred clinical care.
  • Provide clinical leadership, mentoring and support to Registered Nurses, Enrolled Nurses and Care Support Workers.
  • Coordinate assessments, care planning, documentation and clinical reviews.
  • Monitor resident outcomes and ensure compliance with aged care standards and clinical governance requirements.
  • Support medication management, infection prevention and clinical risk management.
  • Promote continuous quality improvement and contribute to accreditation readiness.
  • Foster positive relationships with residents, families and multidisciplinary teams.
  • Support Home Managers in maintaining a positive, collaborative workplace culture.

About You

To be successful in this role, you will have:

  • Current unrestricted AHPRA registration as a Registered Nurse.
  • Demonstrated experience in a Clinical Care Coordinator, Care Manager, Clinical Nurse or senior leadership role within Residential Aged Care.
  • Strong knowledge of the Aged Care Quality Standards and clinical governance.
  • Excellent leadership, communication and coaching skills.
  • Sound clinical assessment and decision-making abilities.
  • Flexibility and willingness to travel and work away from home.
  • Full Australian working rights.
  • Current COVID-19 and other required immunisation compliance (unless medically exempt).
