About the role
As a Care Manager (Full Time), you will be responsible for collaborating with clients, families and stakeholders to deliver person centred care. You will be based in the community with your clients and join us in the office based in Mulgrave and enjoy free onsite parking
Your main duties will include:
- Conduct client visits practicing person centred care planning, offering personalised advice and guidance to clients and/or their families
- Conduct Annual Reviews, revising Care Plans, Goals and service delivery, ensuring budget accuracy and maximum utilisation
- Interacting with internal and external stakeholders
About you
- Clear understanding of Support at Home packages and the recent sector changes, strong understanding of the Aged Care Quality Standards and available community service networks
- Previous or current experience in Care Management within Home Care, EN or RN highly desirable
- Highly organised with the ability to plan, delegate, and manage competing priorities
- Diploma or degree in Health/Allied, or 3+ years' relevant industry experience
- Current Australian driver’s licence and reliable vehicle
