We’re seeking a Clinical Care Liaison—a proactive, compassionate Registered Nurse or Allied Health Professional with excellent communication skills and strong attention to detail—to join our dedicated Statewide PND Service team.

About SPNDS
The Statewide Progressive Neurological Disease Service provides multidisciplinary assessment, education, and management for people with complex progressive neurological conditions including:
- Huntington’s Disease
- Motor Neurone Disease
- Multiple Sclerosis
- Muscular Dystrophy
- Parkinson’s Disease
- and other progressive neurological disorders

About the role
This is a wonderful opportunity to join our Movement Disorders team, working with experienced senior clinicians, including a Nurse Practitioner, as part of a broad multidisciplinary team who have a strong reputation for being supportive, innovative, and exceptional.
The aim of the Movement Disorder Service is to enhance the quality of life of patients and families affected by Huntington’s Disease (HD) and other uncommon Movement Disorders through planning, co-ordinating and delivering the highest level of multidisciplinary care to meet their physical and psychosocial health and disability support needs, throughout the different stages of their life, and death, with the disease.
As a key member of the multi-disciplinary team, the Clinical Liaison will improve continuity of care for patients and their families through their flexible, holistic and collaborative approach to practice, within an outpatient service model.
What your day might look like:
- Attending neurology, psychiatry, and/or palliative care clinic appointments, via telehealth or in clinic face-to-face, to ensure continuity of care
- Arranging follow up referrals, appointments, care plans and management strategies arising from clinic appointments
- Telephone triage and assessment of needs with patients, families, and care providers
- Undertaking interdisciplinary appointments with patients to provide holistic assessment & management
- Providing specialist advice and support to patients, family members, and care providers over the telephone, via telehealth or in face-to-face clinic appointments
- Participating in multi-disciplinary team meetings both internally and with external support providers
- Conducting research activities
What you bring
- Motivation, energy, enthusiasm and the ‘right attitude’, to embrace this interesting and complex patient group will be equally highly valued - even if you don’t have all (or any) of the desirable experience listed below.
- AHPRA registration as a Registered Nurse by the Nursing and Midwifery Board of Australia or Allied Health Professional equivalent based on clinician specialty
- Experience in one or more of: Progressive Neurological Diseases, Palliative Care, Mental Health, Disability, NDIS or Community Health
- Experience in care planning and care coordination
- Proven ability in the management of patients with complex psychosocial, physical and/or mental health needs
- Ability to work independently and autonomously in complex situations, within scope of practice, and respond to unknown and/or unexpected dilemmas
- Ability to understand, respect, and apply principles of dignity of risk to maximise quality of life
- Ability to form therapeutic relationships with patients, families and carers enabling provision of psychosocial and emotional support
- A desire to embrace opportunities for participating in research, education and ongoing quality improvement activities
