About the role
The Palliative Care Social Worker is responsible and accountable for providing a professional level of social work service to the clients of the Calvary Palliative Care Service (Inpatient Palliative Care Unit-PCU and Community Palliative Care Team- CPCT) and to their families and carers. This includes provision of psychosocial assessments, individual and family counselling, service information provision, service linkages and referral and client advocacy.
About you
Degree in social work providing for eligibility for membership of A.A.S.W. with demonstrated ability to practice Social Work Principles of Practice
Demonstrated clinical experience in palliative care, oncology, aged care or other end of life setting including experience working with people from diverse cultural background
Demonstrated ability to perform psychosocial assessments, counselling, discharge planning, home visits and work in a multi-disciplinary team
Knowledge of community services, residential aged care as well as legal and financial issues that may impact on patients, their families and care
Demonstrated high level written and verbal communication and interpersonal skills, and the ability to proactively engage with key stakeholders to enhance service delivery as well as computer skills and the capacity to use health information systems
Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
Current drivers licence providing for eligibility to drive in NSW
Ability to work within the Mission and Values of Calvary Health Care Kogarah
