Regional Intake Coordinator - Home Care | Permanent, Full time | Mulgrave, VIC
Are you passionate about making a meaningful difference in people's lives? Calvary, one of Australia's largest not-for-profit health, community, and retirement care providers, is seeking a dedicated Regional Intake Coordinator to join our Home Care team in Mulgrave.
The role:
As our Regional Intake Coordinator, you'll be the vital link connecting clients with essential home care services across Victoria. You'll manage intake processes, coordinate care assessments, and ensure seamless service delivery that enables clients to live independently and comfortably in their own homes.
Key responsibilities; not limited to:
- Act as the primary point of contact for clients seeking home care services across Victoria
- Coordinate and manage intake processes from initial enquiry through to service commencement
- Conduct comprehensive client assessments to determine care needs and service requirements
- Liaise with multidisciplinary teams to ensure appropriate care planning and service delivery
- Maintain accurate client records and documentation in compliance with regulatory standards
- Collaborate with families, healthcare professionals, and community services to facilitate seamless care transitions
- Monitor and review client progress to ensure ongoing suitability of care packages
- Provide information and support to clients and families regarding available services and funding options
What we are looking for:
- Experience in the aged/disability/community/health sector
- Demonstrated ability to provide quality customer service to a diverse range of prospective customers, families, internal and external stakeholders
- Demonstrated ability to work under pressure
- Well-developed interpersonal and communication skills
- High proficiency with MS Office programs along with demonstrated ability to accurately input data and maintain electronic records. Experience with CRM software preferred.
- Ability to demonstrate innovation and creativity through problem solving and being solutions focused
